CLIENT CHALLENGE

Following an electrical incident involving a contractor at their Warehousing facility, the client required an independent third-party investigation to understand the events leading up to, during, and after the incident. The electrical contractor had experienced an electrical incident whilst working on live electrical equipment at the facility's main distribution panel, raising serious concerns about electrical safety procedures and compliance with the Electricity at Work Regulations 1989.

The client needed a comprehensive investigation that would examine the contractor's documentation, competency records, safety procedures, and working practices.

With potential implications for workplace safety and regulatory compliance, the client sought Bureau Veritas' expertise to provide an impartial assessment of the incident without assigning responsibility, enabling them to understand what had occurred and implement appropriate preventative measures.

The electrical incident occurred during routine maintenance work to relocate circuits from an existing high-level distribution board to a new lower-level installation. The contractor was working at height on step ladders when an electrical incident occurred at the main low voltage panel, highlighting serious concerns about live working procedures and safety protocols.

The client faced significant challenges in understanding how the incident had occurred, particularly given the contractor's claims of competency and the apparent failure of multiple safety systems. Questions arose regarding the adequacy of risk assessments, the appropriateness of live working procedures, and whether proper isolation and testing protocols had been followed.
